  1. The following individuals agree to join a lottery pool for the 
  2. month of ___________________, for the purpose of jointly playing 
  3. the _______ State Lottery:
  4. ___________________________________________________________ and 
  5. the following games of the __________ State lottery:  _________
  6.  
  7. and designate, ______________ as the Lottery Pool Manager for 
  8. this month. Each participant shall be liable to contribute $____ 
  9. per game played.
  10. Each party to the pool shall turn over all funds for the purchase 
  11. of tickets at least 24 hours prior to the closing time for 
  12. purchase of tickets for each game.
  13. In the event that a person does not do so, they shall nevertheless 
  14. be liable to pay in their share of the money used, if a ticket or 
  15. ticket is purchased for them.
  16. In the event that any prizes are won, the pool manager, who shall 
  17. serve without compensation, shall evenly divide the prize among 
  18. the participants, deducting any amount owed for a failure to pay a 
  19. required share. ANY AND ALL FUNDS WON AS PRIZES SHALL BE 
  20. CONSIDERED TO BE HELD IN TRUST FOR THE POOL BY THE MANAGER.  This 
  21. agreement shall not automatically renew and shall expire at the 
  22. end of the month stated.
  23. In the event of a prize being won in excess of $ ______, and 
  24. settlement options are provided, the vote of the majority of the 
  25. pool members shall decide. If there is no majority, the pool 
  26. manager shall decide.
  27. Any member may withdraw by written notice to the pool manager, and 
  28. the pool shall thereupon end, but shall remain in effect for the 
  29. past games played as a pool.

  31. This is the entire agreement of the parties and there are no 
  32. agreements express or implied not stated herein. This agreement 
  33. may only be altered in writing signed by all parties.
